A 32-week, double-blind, up-titration treatment period (from T0 to T4): after screening, patients who meet all eligibility criteria will be randomly assigned to 1 of 2 arms (memantine or placebo) in a 1:1 ratio. Following baseline assessments, each patient will receive a daily dose of memantine or the placebo (up to 20 mg/day).

Research on remote titration of HF medications is somewhat limited; however, the results of previous studies indicate that remote titration could be leveraged to garner improvements in GDMT optimization [13-17].
A pilot RCT was conducted at the Peter Munk Cardiac Centre (PMCC), University Health Network (UHN), in Toronto, between January and December 2019. The trial enrolled 42 patients and compared a remote titration intervention, facilitated by telemonitoring data, with standard in-office titration.

Remote titration of HF medication is a topic that has received little attention despite its potential to contribute to GDMT utilization and optimization.
Several previous trials have investigated remote titration of HF medication with or without the aid of telemonitoring. Two trials by Steckler et al [16] and Moyer-Knox et al [17] assessed remote medication titration over the phone.
